How decentralisation changes life: story of a doctor from occupied territories and Kiptivska AH outpatient clinic

Kostyantyn Kolomiyets and his family moved to the Chernihiv Oblast from the temporarily occupied territories when they realised that it was impossible to work and live there. At first they stayed with friends in the village of the Kozelets Rayon, and then moved to Chernihiv. Kostyantyn and his wife continued to work in the medical sector, their daughter studied at the university. They rented an apartment for a large sum of money. And everyone in the family understood that they needed to look for something different, more promising and long-term.

A general family practice outpatient clinic was opened in the Kiptivska amalgamated hromada in the Chernihiv Oblast in August 2018, before Independence Day. The project included, in addition to treatment rooms, a doctor's apartment as well as a car.

Kipti village head Volodymyr Kuchma said at that time: "We do not have a specialist in the hromada to head the outpatient clinic, but the conditions are proper, so we are looking forward to meeting a doctor." He was quoted by several local publications.

Kostyantyn Kolomiyets read the news and decided to offer his services to the hromada.

The medical system in the Kiptivska AH, past of the ambulance doctor and prospects for improving medical services in rural areas were discussed during a meeting between Oksana Stelmakh, communications adviser of the Chernihiv Local Government Development Centre, established with the support of the U-LEAD with Europe Programme and MinRegion, and Kostyatyn Kolomiyets, head of the Kipti outpatient clinic of general practice of family medicine.

The population of the hromada is about 3,200 residents. As of 1 July, 450 patients had signed declarations with the family practitioner. The AH has an outpatient clinic and five rural health posts maintained by hromada’s own money: they pay salaries to paramedics and nurses.

Kostyantin Kolomiyets started working in the AH in March, since the first days there have been many people wishing to make an agreement with the doctor. Those who are ill or have health problems continue to sign declarations.

Patients in the outpatient clinic can consult with a family doctor, get prescription, here they can check pressure and make a cardiogram, have drips put, get vaccinations and injections. There is a physiotherapy room and two inpatient treatment beds. In remote villages, Kostyantyn conducts field visits and consultations according to the schedule.

The doctor's wife is a nurse who also works at the clinic. The hromada provided housing to the family. Salary depends on the number of signed declarations and patients. Therefore, the life in a new place is slowly getting better and one can think about the future.

The medical facility is equipped with a specific set of tools and equipment, the legislation envisages a list of equipment necessary for the outpatient clinic to have. Therefore, the AH is working hard to meet the requirements. Here, they work to provide residents with quality health services.

Hromada residents are happy with the doctor. “It is very good that this hospital was opened. There is a person to apply to. Drips can be put, medicines prescribed, and the pharmacy works here as well. The doctors that came are professional. This is also a contribution of the village council,” said Anna Hanyailo, resident of the Kiptivska AH.
